In the game, which is about the Cold War period, events begin to develop when the Murkoff Corporation recruits people as guinea pigs to conduct mind control tests. While these tests, which are said to be carried out in the name of progress and science, reach incredible levels, the fear thresholds of the players are also tested.

In order to escape conviction and reintegrate into society, you must operate by resisting Murkoffs therapy. For this, apart from the tests, you also need to complete the MK-Challenges. While the trials in the game consist of story-based immersive therapies, MK-Challenges refer to relatively shorter therapies that occur as a result of changing the existing maps.

The Outlast Trials has a concept that you can play as Co-Op alongside the story-oriented games of the series. In the game, where you all start as Murkoff Corporation prisoners, you can try to complete the experiments alone or in a group.
